All patient appointments, forms, and payments can be managed online in your secure portal - to make your visit as seamless as possible. Plus, while we don’t contract directly with all insurance providers, we will work with your insurance on your behalf to remove a to-do from your list.

Before you come in, you are able to complete all of your forms online. One less thing to think about on appointment day. Insurance
Not in-network? No insurance? No problem.
Mt Tam Pediatric Dentistry works with all PPO plans and regardless of what PPO plan you have, we’ll take care of all insurance claims and billing on your behalf. Additionally, when requested, we are happy to work with your insurance provider to find out what your plan covers prior to your initial visit. Some out-of-network PPO plans reimburse 100% of our fees for preventative services and up to 50-80% on everything else. Reach out if you’d like to explore your plan benefits!
To request an estimate of how much your plan reimburses, please text a “request for pre-auth” to 415.329.6780. Include a picture of your insurance card, your child’s age, as well as the first and last name, date of birth, and address of the enrollee.
If you’re uninsured, your plan doesn’t provide coverage for out-of-network providers, or your PPO plan has limited benefits or reimbursement, the Tam Teeth Club may be for you. Read below to learn more about our membership program.
